  1. I give my permission to receive a spa service from Spa Theory and the provider performing the service.

  2. I release Spa Theory and the provider performing my service of any liability associated with any injuries and /or current and future conditions resulting from the procedures or products.

General Liability Release Form: Massage Consent

By clicking the button “Consent to Spa Services” you agree to the following:

  1. I give my permission to receive massage therapy.

  2. I understand that therapeutic massage is not a substitute for traditional medical treatments or medications.

  3. I understand that the massage therapist does not diagnose illnesses or injuries, or prescribe medications. 

  4. I have clearance from my physician to receive massage therapy.

  5. I understand the risks associated with massage therapy, including but not limited to: superficial bruising, short-term muscle soreness, exacerbation of undiscovered injuries. I therefore release Spa Theory and the massage therapist performing the massage from all liability concerning these injuries that may occur during the massage session. 

  6. I understand the importance of informing my massage therapist of all medical conditions and medications I am taking, and to let the massage therapist know about any changes to these. I understand there may be additional associated risks to these. 

  7. I understand it is my responsibility to inform my massage therapist of any discomfort I may be feeling during the massage session so they may adjust accordingly. 

  8. I understand that I, or the massage therapist, may terminate the massage session at any time. 

  9. I release Spa Theory and the massage therapist performing my service of any liability associated with any injuries and /or current and future conditions resulting from the procedures or products.


General Liability Release Form: Facial Consent

By clicking the button “Consent to Spa Services” you agree to the following:

  1. I acknowledge that my skin might experience temporary irritation, tightness, redness or slight swelling which usually dissipates within 72 hours depending on skin sensitivity. 

  2. I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions.

  3. I acknowledge that if I fail to use a minimal sunscreen (SPF45), I am more susceptible to sunburn, skin damage & hyperpigmentation. I should avoid excessive sun exposure especially between 10am-2pm.

  4. I acknowledge that this treatment is strictly an elective cosmetic procedure and no medical claims have been expressed or implied.

  5. I acknowledge that I should avoid the use of Retin-A type products, aggressive exfoliation, waxing, and products containing acids for 2-4 weeks following treatment.

  6. I consent (to the best of my knowledge) that the answers I have given are correct and that I have not withheld any information that may be relevant to my treatment. 

  7. I understand that I, or the esthetician, may terminate the session at any time. 

  8. I release Spa Theory and the esthetician performing my service of any liability associated with any injuries and /or current and future conditions resulting from the skincare procedures or products.


General Liability Release Form: Waxing Consent

By clicking the button “Consent to Spa Services” you agree to the following:

  1. I acknowledge that some possible side effects include, but are not limited to, redness, swelling, tenderness, and pimples.

  2. I understand that a waxing treatment does not treat medical conditions nor does it claim or guarantee to treat or relieve any medical condition.

  3. For Brazilian and/or bikini waxing, I will notify my service provider if I am on my menstrual cycle.

  4. I do not have any open skin lesions or active herpes outbreak (cold sore or genital).

  5. I agree to adhere to all safety post care including: no peels, tanning or wet room services; no swimming/spas/hot tubes for 24-48 hours after waxing; and all home skin care protocols as recommended by my service provider.

  6. I will wear clean, loose fitting clothes.

  7. I will avoid swimming in chlorinated pools.

  8. I understand that I, or the waxing specialist, may terminate the session at any time. 

  9. I release Spa Theory and the waxing provider performing my service of any liability associated with any injuries and /or current and future conditions resulting from the procedure or products.
